  • Should you wish to make a complaint against an ACHA member, please do so on ACHA Complaint Form. 
  • Open the form by clicking on the link immediately below. The pdf file should open in your browser window. 
  • You will find the form on the final page.
  • Please fill in the details with your computer before printing. Use your mouse to click in each area and fill in the requested details. Once you are satisfied that your information is correct, then print.

Once it has been filled in and printed:

  • Sign the form
  • Date the form
  • Make a photocopy of the final page (for your records)
  • Post the original to the ACHA. The address is on form
